| Video Discription | KEEPING PEOPLE HEALTHY AWARD This award is presented to a team that promotes good health through awareness of healthy choices, preventing and addressing poor health, and works to improve the overall health and wellbeing of the community. FINALIST Collaboration was key when the Murrumbidgee Aged Care Team developed an Efficiency Improvement Project (EIP) in partnership with Clinical Operations to increase the district’s use of Transitional Aged Care Packages (TACP) and empower older people to spend healthier days at home. TACP is a 12 week program designed to help older people recover after a hospital stay. It provides short-term specialised care and support to help the patient regain their functional independence and confidence sooner, and avoid the need for longer term care and support services. By showcasing and emphasising the objective that also aligns with the Districts strategic goals, the integrated operations teams have increased TACP use across our region to a consistent utilisation of less than 95%. This achievement is not only getting State recognition and praise, but is allowing our older communities to safely recover in the comfort of their own homes. |
