In patients with small bowel obstruction, supine views show dilation of multiple loops of the small bowel, with a paucity of air in the large bowel. These findings, in conjunction with a lack of air and stool in the distal colon and rectum, are highly suggestive of mechanical intestinal obstruction.
Dilated small bowel loops more than 2.5 cm up from outer wall to outer wall; normal caliber or collapsed loops distally are signs of intestinal obstruction.
The relevance of intestinal free fluid between loops detected by sonography in the clinical assessment of small bowel obstruction in adults is significant.
The presence of a large amount of fluid between dilated small bowel loops suggests worsening of mechanical small bowel obstruction, that requires not medical therapy but immediate surgery.
Obstruction of the large bowel usually causes milder symptoms that develop more gradually than those caused by small-bowel obstruction. Increasing constipation leads to obstipation and abdominal distention. Vomiting may occur (usually several hours after the onset of other symptoms) but is uncommon.
