| Video Discription | Prior to availability of the vaccine in 1963, measles affected 3-4 million children each year. Complications resulted in more than 45,000 hospitalizations leaving 1000 children with chronic disabilities from measles encephalitis and causing more than 500 deaths each year. The vaccine almost vanquished the disease except in those who opt against it. Worldwide more than 20 million cases of measles will occur this year with a death toll exceeding 90,000. Measles spreads through the air launched by sneezing, coughing or even merely breathing. The virus remains airborne for up to 2 hours after an infected individual leaves the room. Droplets landing on doorknobs, light switches and countertops remain infectious. On contact, the likelihood of infection to the unvaccinated averages 90%. Infected individuals are contagious 4 days before the rash begins through 4 days after it appears. After an incubation of 10-14 days, the first manifestations may be a fever as high as 104 degrees eventually accompanied by cough, conjunctivitis and coryza – runny nose. White spots fleetingly appear on the inner lining of the cheeks near the molars and then in several days the typical rash develops. The cutaneous eruption of measles starts on the head and eventually spreads from the face to the torso and finally the extremities. Generally itchy the non-specific appearing rash mimics a drug eruption or a routine viral rash. After several days to a week it fades with complete recovery in about 10 days. Complications include ear infection, bronchopneumonia, laryngotracheobronchitis, diarrhea, hearing loss and corneal ulcers potentially leading to vision loss. Encephalitis leading to permanent brain damage, seizures and death may result from infection. People most at risk for untoward reactions include those less than age 5 or older than age 20, the malnourished, those living in crowded conditions and pregnant women. Immunocompromised individuals with HIV/AIDS and malignancies are at dramatically increased potential for harm. Supportive treatment, fever control, rehydration and management of any secondary bacterial infection remain the only available options. Two doses of the live attenuated measles vaccine are recommended. The first inoculation should be administered between age 12-15 months with the second between age 4-6 years. At present in the United States it is available only in combination as the MMR - measles, mumps, rubella - vaccine. After the second dose, lifelong protection exists. The vaccine does not cause autism, Guillain-Barre syndrome, inflammatory bowel disease or other long term sequelae. Since the vaccine contains live although weakened virus, caution appears warranted for those with HIV/AIDS, leukemia or lymphoma. The same applies to those undergoing cancer chemotherapy or receiving high dose steroids. |
