This muscle attaches from the anterior surface of S2-S4 and travels through the greater sciatic foramen to the greater trochanter and is an extensor, abductor and lateral rotator of the hip joint. The piriformis has a relationship to the sciatic nerve and is considered that this nerve can travel through the muscle in 20% of the population and cause a sciatic type of symptoms called a piriformis syndrome, for the majority the nerve will pass underneath this muscle.
