This is what we term as recurrent implantation failure, when good quality embryos don’t implant , then everybody starts worrying as to what the problem could be. The problem could be in the embryo or in the uterus. Most importantly there could be problem in the embryo itself and when it comes to the embryo, it could be a problem with the oocyte or the sperm or it can be problem in the combination of the two. There could be increased DNA fragmentation in sperm which reduces chances of implantation, there can be increased cytokine activity or inflammatory activity which reduce the chances of implantation . There could be increased cytokine activity inflammatory activity that reduces the chance of embryo implantation. This can happen in conditions like endometriosis or in women with low AMH levels and it is not that if one embryo has the problem then all the embryos will have the same. Luckily every embryo is different because every embryo is made of a different composition. There can be mismatch causing the embryo and causing the embryo not normal enough for implantation to happen. it is natures mechanism to reject embryos that are not worth implanting into the uterus and that is the reason why abnormal embryos don’t usually implant and subnormal embryos do implant and either carry forwards for a very few weeks of gestation ad result in very early stages of pregnancy called as biochemical pregnancy. In cases of implantation failure due to the uterus not supporting the embryo, there can be reduced blood supply to the lining of the uterus because of which there can be reduced implantation chances and sometimes there can be elevated levels of cells in the uterus that will reduce the chances for a good quality or a normal quantity too implant. There can also be auto antibodies against the oocyte and uterine lining because of which there are reduced implantation chances and sometimes we also notice there can be physical problems inside the uterus like endometrial polyps, fibroids, there can be adhesions in the uterus because the previous curettages or infection inside uterus and this is another cause or pregnancy not happening or implantation not happening. Sometimes there can be lot of HLA matching because of which the egg fails to implant. It can also be a common genetic condition because of which he husband and the wife have and because of which the embryo that is formed has a major genetic abnormality and that is the major reasons why there can be an implantation failure or a pregnancy. So recurrent implantation failure is matter of great concern and most of the times it is difficult to tap the cause and if one ha s to answer how to treat implantation failure it goes to understand what are the reasons and to increase the chances of the implantation.
